Fragrance-Free Clinic Policy

Many of our patients are highly sensitive to various environmental factors. In order to provide surroundings suitable for their needs, we have instituted a few clinic policies and requirements for all patients both before and after their arrival at our clinic. Your adherence to the policies listed below as well as any sacrifices necessary on your part will be greatly appreciated by all.

It is required that all who enter the clinic should refrain from using or wearing scented personal care products such as:

  • Scented hand soap
  • Scented laundry detergent & fabric softener
  • Perfume & scented aftershave
  • Scented shampoo, conditioner, and hair styling products
  • Scented deodorant
  • Lotion with fragrance
  • Scented cosmetics & powders

Patients, family members, and guests should refrain from smoking the day of the clinic visit, as smoke clings to you, your clothes, and personal belongings. If you or anyone accompanying you to your appointment cannot go without tobacco that long, at least one hour should elapse before entering the clinic, or they might consider using a nicotine gum or patch.

Please complete these forms and bring them with you before your first visit to our clinic. This will speed up the check in process and give the doctor the information they need to assess your health.

Financial Policies

Medicare and Insurance:

We only file for Medicare and Tricare insurance plans. With Medicare Alternative plans, we charge the patient the full amount of the Medicare rate. If the patient has a secondary insurance plan in addition to Medicare or Tricare, the patient must set up the crosswalk between their insurance plans. Our office staff is unable to do this on behalf of the patient. If a service is not covered by Medicare or Tricare, the patient must pay out of pocket at the time of service.

All other patients who do not have Medicare or Tricare must pay out of pocket for all services rendered at the time of service. We will provide them with the appropriate documentation for them to file with their insurance for reimbursement. At this time our staff is unable to file insurance on behalf of patients.

Returns and Refunds:

There are no returns or refunds of any kind on lab work or lab kits. All unopened supplements may be returned within 30 days of purchase.
